Job Description

We are seeking a Case Manager to join our team. You will help families and households navigate their challenges and create integrated support plans. This role focuses on safety, coordination, and team development. Key Responsibilities Map out the situation, conducting a sharp problem analysis with safety as a priority. Develop and implement integrated plans with families, networks, and professionals using the 1family1plan1director method. Arrange necessary care and support under the Youth Act and Social Support Act (Wmo). Maintain an overview of goal progress through regular meetings with all stakeholders. Lead family network meetings, record agreements, and evaluate plans. Consult with Access Unit colleagues and partners to adjust plans as needed. Contribute to the development of the Case Management team. Required Qualifications Completed higher professional education. Several years of relevant work experience with complex parenting, developmental issues, or adults with care-avoiding behavior. SKJ registration, or ability to obtain it within a short timeframe. Up-to-date knowledge of the Youth Act, Social Support Act (Wmo), and related laws. Strong organizational, reporting, and communication skills. Requirements
